PoliciesPatient InformationIt is important for us to have the most current information on our patients. Without up-to-date information, we cannot communicate with you regarding lab results, refills, etc. Thus, a new Patient Information Form must be filled out annually. This form is also a legal document giving us permission to treat the patient, bill your insurance, and provide medical information to other providers. Your signature and a completed form is imperative. We apologize for any inconvenience, but this process is necessary for us to effectively care of you as our patient. Lab ResultsRoutine lab and x-ray tests will be reported within 7 to 10 days by phone regardless of what the lab or x-ray department may tell you. Pap smear results will be made available to the patient within 14-21 days. The lab will submit to you a separate fee for pathology services not covered by insurance. Please do not call our office for test results unless there is a change in your condition, or if you have not heard from us in 2 weeks. Should your results indicate that you require urgent treatment, you will be notified immediately. RefillsPlease do not call our office for refills. We must receive a refill request from your pharmacy. Please call in your refill request to your pharmacy 2 to 3 days before your medication is depleted to allow time for the request to be approved. Please allow 48 hours for mail order prescriptions to be written. Return Phone CallsThe nurses are not available to take or return phone calls until after we have finished seeing patients for the day. Calls considered an emergency can be left with the receptionist and a nurse will return your call.
