InsuranceCo-pays/Co-insurancePlease be prepared to pay your co-pays at every visit. If you have not met your deductible, you will be responsible for the day's charges until your deductible requirements have been fulfilled. Also, after we have filed your insurance, there may be a portion of the charges that are your responsibility after contractual adjustments have been taken. Insurance VerificationIt is your responsibility to be sure that we are providers on your health plan. Please look to your participating provider manual supplied to you by your plan, or call your plan to verify this information. If we are not providers with your health plan, you will be responsible for all charges not paid by your insurance. When you arrive for each visit, you will be asked to present a copy of your insurance card. Without proof of your insurance at each and every visit, we cannot confirm your coverage and bill your insurance for you. If you do not have the card with you, you will be required to pay for your visit and we will provide you with an encounter form at that time so that you may file your own insurance. If your insurance information has changed, you must notify our office and present your new insurance card to us at each visit. You will be responsible for all charges if our records do not reflect your current insurance information.
